Informace o kvalifikační práci Celkový hmotnostní přírůstek v těhotenství a jeho vliv na vybrané těhotenské, porodnické komplikace a hmotnost novorozence
Úvod: Diplomová práce se zabývá hmotnostními přírůstky v těhotenství. Zkoumá faktory ovlivňující výskyt nadměrného a nedostatečného hmotnostního přírůstku. Dále se zabývá důsledky nadměrného a nedostatečného hmotnostního přírůstku.
Cíl: Zmapovat faktory ovlivňující výskyt nadměrného a nedostatečného hmotnostního přírůstku. Zjistit do jaké míry má nadměrný a nedostatečný hmotnostní přírůstek vliv na výskyt makrosomických, hypertrofických, hypotrofických novorozenců a novorozenců s nízkou porodní hmotnostní, taktéž na výskyt císařského řezu a gestačního diabetu.
Metoda: Výzkumné šetření bylo provedeno pomocí kvantitativního typu výzkumu. Výzkumné šetření probíhalo v období od září do října 2021 prostudováním příslušné dokumentace. Výzkumný soubor tvořilo 496 žen ve věku od 18 let do 46 let, s termínovou graviditou od týdne 37+0 a jednočetným těhotenstvím. Pomocí statistických metod jsme ověřovali jednotlivé hypotézy a plnili hlavní cíl a dílčí cíle práce.
Výsledky: Bylo prokázáno, že existuje statisticky významný rozdíl v zastoupení u jednotlivých kategorií hmotnostního přírůstku (nedostatečný, doporučený, nadměrný) podle BMI, parity a vzdělání. Nebylo prokázáno, že existuje statisticky významný rozdíl v zastoupení v jednotlivých kategorií hmotnostního přírůstku podle věkových kategorií. Dále bylo zjištěno, že je statisticky významný rozdíl v četnostním zastoupení v jednotlivých kategoriích hmotnostního přírůstku u makrosomických, hypertrofických novorozenců a novorozenců s nízkou porodní hmotnostní a u žen s gestačním diabetem. Nebyl zjištěn statisticky významný rozdíl v zastoupení v jednotlivých kategoriích hmotnostních přírůstků u hypotrofických novorozenců a porodů císařským řezem.
Závěr: Práce potvrdila vliv parity, vzdělání a BMI na výskyt nadměrného a nedostatečného hmotnostního přírůstku v těhotenství. Vliv věku se nepotvrdil. S pomocí znalosti těchto faktorů se mohou poskytovatelé zdravotní péče zaměřit na ženy, které jsou více ohroženy nadměrným a nedostatečným hmotnostním přírůstkem. Z výsledků výzkumného šetření je patrné, že nadměrný a nedostatečný hmotnostní přírůstek jsou proměnné, jejichž vliv se uplatňuje na výskyt makrosomie, hypertrofie, nízké porodní hmotnosti a gestačního diabetu. Nepotvrdili jsme vztah mezi nedostatečným hmotnostním přírůstkem a vyšším výskytem hypotrofických novorozenců a mezi nadměrným hmotnostním přírůstkem a vyšší četností císařského řezu.

Introduction: The thesis deals with weight gain in pregnancy. It examines the factors influencing the occurrence of excessive and insufficient weight gain. And assesing the effect of gestational weight gain during pregnancy and adverse maternal and fetal outcomes.
Aim: To evaluate the factors influencing the incidence of excessive and insufficient weight gain. To find out to what extent excessive and insufficient weight gain affects the incidence of macrosomic, large for gestational age, small for gestational age and low birth weight newborns, caesarean section and gestational diabetes.
Method: The research investigation was conducted using quantitative type of research. The research investigation was carried out between September and October 2021 by studying relevant medical records. The research population consisted of 496 women aged 18 years to 46 years, from 37+0 weeks of pregnancy and with singleton pregnancy. Statistical methods were used to test the hypotheses and meet the sub-objectives.
Results: It was shown that there was a statistically significant difference in the representation in each weight gain category (insufficient, recommended, excessive) according to BMI, parity and education. There was no evidence of a statistically significant difference in representation in each weight gain category by age. We also found that there was a statistically significant difference in frequency representation in each weight gain category for macrosomic newborns, large for gestational age, low birth weight newborns and women with gestational diabetes. We did not find a statistically significant difference in the representation of each category of weight gain in small for gestational age newborns and cesarean deliveries.
Conclusion: This thesis confirmed the effect of parity, education and BMI category on the prevalence of excess and insufficient weight gain in pregnancy. The effect of age was not confirmed. By knowing these factors, health care providers can target women who are at risk of excessive and insufficient weight gain. It is evident from the results of the research that excessive and insufficient weight gain are variables whose influence is exerted on the incidence of macrosomia, large for gestational age, low birth weight and gestational diabetes. We did not confirm a relationship between insufficient weight gain and a higher incidence of small for gestational age and between excessive weight gain and a higher rate of caesarean section.
