Úvod: Srdeční selhání patří mezi nemoci nejvíce zatěžující pacienty i zdravotní systém. Jedním z pilířů léčby je farmakoterapie, u níž je podmínkou efektivity adherence.
Metodika: Vytvořili jsme unicentrický registr pacientů se stabilním chronickým srdečním selháním o dvou kohortách s časově odděleným náborem (roky 2018 a 2020). U všech pacientů jsme změřili sérové hladiny léků (především ACE inhibitorů, sartanů, ARNI, betablokátorů a MR antagonistů) jako metodu přímé detekce adherence. Dále jsme sledovali klinické, biochemické a u části pacientů i zátěžové parametry.
Výsledky: V obou kohortách dohromady bylo 81 % pacientů plně adherentních k farmakoterapii srdečního selhání. Kohorta z roku 2020 vykazovala ve srovnání s kohortou z roku 2018 statisticky významně (p <0,01) nižší adherenci (74,2 % oproti 84,25 %).
Většina klinických parametrů mezi oběma kohortami zůstala konzistentní. Došlo však k malému, statisticky významnému zvýšení BMI a lačné glykémie. Non-adherence byla v kohortě z roku 2018 pravděpodobnější u pacientů mladších 60 let (p = 0,018). Mezi roky 2018 a 2020 jsme zaznamenali významný posun od používání inhibitorů ACE k ARNI.
Subjektivní a objektivní funkční kapacita pacientů se srdečním selháním se lišila, zejména na obou koncích spektra. Rozložení VO2 max podle příslušných kategorií NYHA vedla ke ztrátě informace, vhodnější bylo používat přímo VO2 max. Naše data neposkytla důkazy o velkých rozdílech v subjektivně-objektivních trendech ve funkční kapacitě mezi adherentními a nonadherentními pacienty.
Ke zkoumání vztahu mezi zdravotním stavem a adherencí k lékům jsme vytvořili statistický model nazvaný "Badhealth". Model naznačuje, že horší zdravotní stav je asociován se zvýšenou adherencí.
Závěr: Testování hladiny léků v séru k posouzení adherence pacientů lze použít u pacientů se srdečním selháním v běžné klinické praxi. Využití ukazatelů integrujících subjektivní funkční kapacitu a objektivní parametry závažnosti srdečního selhání by mohlo zajistit větší přesnost posuzování úrovně adherence k medikaci při klinickém sledování.

Introduction: Heart failure is one of the most burdensome diseases for both patients and the healthcare system. One of the pillars of treatment is pharmacotherapy, for which adherence is a prerequisite for effectiveness.
Methods: We created a unicentric registry of patients with stable chronic heart failure with two cohorts with time-separated recruitment (2018 and 2020). Serum drug levels (mainly ACE inhibitors, AT1-blockers, ARNI, beta-blockers and MR antagonists) were measured in all patients as a direct method to detect adherence. In addition, we monitored clinical, biochemical and, in some patients, exercise parameters.
Results: In both cohorts, in total 81% of patients were fully adherent to heart failure pharmacotherapy. The 2020 cohort had statistically significantly (p <0.01) lower adherence rates (74.2% versus 84.25%, respectively) compared to the 2018 cohort.
Most clinical parameters remained consistent between the two cohorts. However, there was a small, statistically significant increase in BMI and fasting glycaemia. Non-adherence was more likely in the 2018 cohort among patients younger than 60 years (p = 0.018). Between 2018 and 2020, we saw a significant shift from the use of ACE inhibitors to ARNI.
Subjective and objective functional capacity of heart failure patients varies, especially at both ends of the spectrum. The distribution of VO2 max according to the relevant NYHA categories leads to a loss of information, it is preferable to use VO2 max directly. Our data do not provide evidence for large differences in subjective-objective exercise capacity trends between adherent and nonadherent patients.
To investigate the relationship between health status and medication adherence, we developed a statistical model called "Badhealth". The model suggests that poorer health status is associated with increased adherence.
Conclusion: Testing serum drug levels to assess patient adherence can be used in patients with heart failure in routine clinical practice. The use of integration of subjective functional capacity and objective parameters of heart failure severity could provide greater accuracy in assessing the level of medication adherence during clinical follow-up.
