Úvod: Endometrióza je jedním z nejčastějších gynekologických onemocnění postihující ženy v reprodukčním věku. Hlavním příznakem je bolest, která může mít výrazný dopad na život ženy. Má rozsáhlé psychické, fyzické a sociální dopady na život pacientky a negativně ovlivňuje kvalitu života související se zdravím. Vliv léčby tohoto onemocnění na kvalitu života žen v České republice však dosud nebyl pomocí specifického nástroje zkoumán.
Cíl: Hlavním cílem práce bylo zjistit, jaký vliv má onemocnění endometrióza na kvalitu života. Dílčími cíli bylo pomocí dotazníku Endometriosis Health Profile (EHP-30) a 36-Item Short Form Health Survey Questionnaire (SF-36) srovnat kvalitu života žen s endometriózou před léčbou a za 3 měsíce po zahájení léčby. Dále vyhodnotit bolest a její intenzitu, jako jeden z hlavních příznaků tohoto onemocnění a zjistit vztah mezi vybranými faktory a kvalitou života žen s endometriózou.
Soubor a metodika: Longitudinální výzkum se uskutečnil v letech 2020-2023 na dvou pracovištích zabývajících se léčbou endometriózy (Fakultní nemocnice Olomouc a Nemocnice Frýdek - Místek, p. o.). Zúčastnilo se celkem 38 pacientek ve věku 21 - 49 let, z nichž 26 pacientek podstoupilo chirurgickou léčbu, 6 farmakologickou a 6 chirurgickou i farmakologickou. K hodnocení kvality života byly použity dotazníky EHP-30 (specifický dotazník pro hodnocení kvality života žen s endometriózou) a SF-36 (generický dotazník pro hodnocení celkové kvality života) v české verzi a numerická škála měření bolesti (Numeric Rating Scale-NRS). Dotazníky byly ženám předkládány dvakrát, před zahájením léčby a následně za 3 měsíce. Pro zpracování výsledků byla použita popisná statistika a ze statistických metod Shapiro-Wilkův test, Wilcoxonův test, McNemarův test a Spearmanův korelační koeficient. Všechny testy byly provedeny na hladině signifikance 0,05.
Výsledky a diskuze: Při porovnání kvality života dotazníkem EHP-30 před léčbou a za 3 měsíce bylo zjištěno signifikantně lepší hodnocení kvality života ve všech doménách kromě domény "Neplodnost" (p = 0,739). Pomocí dotazníku SF-36 jsme zjistili, že kvalita života žen se statisticky významně zvýšila ve všech oblastech života kromě domény "Zdravotní změny" (p = 0,774). Dále výsledky prokázaly, že za 3 měsíce po léčbě mají ženy statisticky významně vyšší průměrnou hodnotu celkového skóre dotazníku SF-36 hodnotící kvalitu života (78,11) než před léčbou (63,74). Hodnocení bolesti pomocí NRS ukázalo statisticky významné zlepšení bolesti při menstruaci, mimo menstruaci, při pohlavním styku, při mikci i při defekaci za 3 měsíce po léčbě. Snížení bolesti vedlo k lepšímu hodnocení kvality života v rámci některých domén dotazníku EHP-30 a SF-36. Prokázal se i významný vliv věku, vzdělání a počtu porodů a potratů na kvalitu života v některých doménách.
Závěr: Léčba endometriózy vede k subjektivnímu snížení intenzity bolesti jako jednoho z hlavních příznaků tohoto onemocnění a zlepšuje kvalitu života žen s endometriózou. Prevalence onemocnění v České republice za poslední roky narůstá, proto je nutné se onemocněním zabývat a včas zahájit vhodnou léčbu, která může významně zlepšit kvalitu života žen trpících tímto onemocněním a snížit také dopady pro společnost a finanční náklady. Edukace žen a dívek a otevřená komunikace zdravotníků může přispět k lepší informovanosti žen, včasnému vyhledání odborné pomoci a zahájení léčby.

Introduction: Endometriosis is one of the most common gynaecological diseases affecting women of reproductive age. The main symptom is pain, which can have a significant impact on a woman's life. It has widespread psychological, physical, and social impact on the patient's life and negatively affects health-related quality of life. However, the impact of treatment of this disease on the quality of life of women in the Czech Republic has not yet been investigated using a specific tool.
Aim: The main aim of the study was to investigate the impact of endometriosis on quality of life. The sub-objectives were to compare the quality of life of women using the Endometriosis Health Profile (EHP-30) and the 36-Item Short Form Health Survey Questionnaire (SF-36) with endometriosis before treatment and 3 months after treatment. Furthermore, to evaluate pain and its intensity as one of the main symptoms of this disease and to determine the relationship between selected factors and quality of life of women with endometriosis.
Sample and methodology: The longitudinal research was conducted in the years 2020-2023 at two institutions dealing with the treatment of endometriosis (University Hospital Olomouc and Hospital Frýdek - Místek, p. o.). A total of 38 patients aged 21-49 years participated, of whom 26 underwent surgical treatment, 6 pharmacological treatment and 6 both surgical and pharmacological treatment. The questionnaires EHP-30 (specific questionnaire for assessing the quality of life of women with endometriosis) and SF-36 (generic questionnaire for assessing the overall quality of life) in the Czech version and the Numeric Rating Scale-NRS (Numeric Rating Scale for pain) were used to assess the quality of life. The questionnaires were administered twice to the women, before the start of treatment and subsequently at 3 months. Descriptive statistics were used to process the results and statistical methods included Shapiro-Wilk test, Wilcoxon test, McNemar test and Spearman correlation coefficient. All tests were performed at 0.05 level of significance.
Results and Discussion: Using the SF-36 questionnaire, we found that women's quality of life was statistically significantly increased in all domains of life except for the domain "Health Changes" (p = 0.774). Furthermore, the results showed that at 3 months post-treatment, women had a statistically significantly higher mean value of the total score of the SF-36 questionnaire assessing quality of life (78.11) than pre-treatment (63.74). Pain assessment using NRS showed statistically significant improvement in pain during menstruation, non-menstruation, intercourse, micturition, and defecation at 3 months post-treatment. Reduction in pain led to better quality of life scores in some domains of the EHP-30 and SF-36 questionnaire. There was also a significant effect of age, education, and number of births and abortions on quality of life in some domains.
Conclusion: The prevalence of the disease in the Czech Republic has been increasing over the last years, therefore it is necessary to address the disease and to initiate timely and appropriate treatment, which can significantly improve the quality of life of women suffering from this disease and also reduce the impact on society and financial costs. Education of women and girls and open communication between health professionals can contribute to better information for women, timely seeking of professional help and initiation of treatment.
