Cíl práce: Ventilátorová pneumonie (VAP) je vůbec nejčastější nozokomiální infekcí v intenzivní péči. Recentní prameny zmiňují vliv předchozího užití antibiotik (ATB) zejména v souvislosti s léčbou komunitních infekcí na epidemiologii VAP. Primárním cílem studie bylo zhodnocení vlivu předchozí aplikace ATB na incidenci VAP, 30denní mortalitu a spektrum etiologických agens VAP, včetně výskytu multirezistentních (MDR) bakteriálních patogenů. Hodnotili jsme však vliv veškeré aplikace ATB předcházející vzniku VAP, a to včetně profylaxe a léčby nozokomiálních infekcí. V následné studii klonalit byla ověřena hypotéza, zda se v etiologii VAP uplatňují etiologická agens z bakteriální mikroflóry horních cest dýchacích výrazněji, resp. dochází častěji k vertikálnímu šíření patogenů než k jejich horizontálnímu šíření mezi pacienty.
Materiál a metody: Do této retrospektivní observační studie byli zařazeni pacienti splňující klinická kritéria VAP. Kontrolní skupinu tvořili nemocní vyžadující umělou plicní ventilaci déle než 48 hodin bez známek pneumonie. Obě skupiny byly porovnány podle předchozí aplikaci ATB. Stěry z orofaryngu a endotracheální aspirátu (ETA) odebrané u všech pacientů byly zpracovány standardními mikrobiologickými postupy. Citlivost k ATB byla stanovena dle doporučení European Committee on Antimicrobial Susceptibility Testing (EUCAST). Klonální vztahy bakteriálních izolátů byly stanoveny porovnáním jejich makrorestrikčních profilů získaných gelovou elektroforézou v pulzním poli (PFGE). Restrikční profily dosahující 97% podobnosti byly považovány za identické.
Výsledky: Celkem bylo zařazeno 104 pacientů. Kritéria VAP splnilo 47 pacientů (14 pacientů s předchozím užíváním ATB, 33 pacientů bez terapie). Četnost VAP byla u pacientů s předchozí aplikací ATB nižší (36 %) oproti skupině bez předchozí aplikace ATB (51 %), (p = 0,159). Nejčastěji zachycenými patogeny byly enterobakterie, včetně kmenů produkujících širokospektré beta-laktamázy (ESBL, AmpC), a kmeny Pseudomonas aeruginosa. Podíl MDR patogenů se v závislosti na předchozí aplikaci antibiotik však statisticky nelišil (MDR po předchozí ATB 71 % vs. MDR bez předchozí aplikace ATB 64 %, p = 0,742). 30denní mortalita VAP s předchozí aplikací ATB byla nižší, bez statistické významnosti (p = 0,688). Do studie vyšetření klonality bylo zařazeno 20 pacientů, u nichž byla VAP vyvolána kmeny Klebsiella spp. a Escherichia spp. Celkem bylo získáno 49 izolátů. U 75 % byl zachycený bakteriální patogen, jako etiologické agens VAP, izolován z horních cest dýchacích před rozvojem VAP nebo bakteriální kmeny identifikované ze vzorků horních cest dýchacích a ETA vykazovaly identické restrikční profily u daného pacienta.
Závěr: Předchozí podávání ATB je spojeno s nižší incidencí VAP a současně mírně vyšším výskytem MDR bakteriálních kmenů. 30denní mortalita pacientů VAP s předchozí aplikací ATB byla mírně nižší. Vysoká variabilita bakteriálních patogenů izolovaných od různých pacientů svědčí pro převažující endogenní původ infekce VAP. Znalost předchozí aplikace antibiotik a výsledků mikrobiologických nálezů může časně modifikovat antibiotickou léčbu VAP a příznivě ovlivnit 30denní mortalitu pacientů.

Objectives: Ventilator-associated pneumonia (VAP) is the single most common healthcare-associated infection in intensive care. Recent literature mentions the effect of previous antibiotic therapy on VAP epidemiology mainly in association with communityacquired infections. The primary objective of the study focuses on evaluating the effect of previous antibiotic therapy on the incidence of VAP, 30-day mortality and the spectrum of etiologic agents casing VAP, including multidrug-resistant (MDR) bacterial pathogens. However, we aimed to determine the effect of all antibiotics used in the pre-VAP period, including prophylaxis and healthcare-associated infection therapy. Subsequently, clonality was studied to verify a hypothesis that bacteria originating from upper respiratory tract flora play a more prominent role in the etiology of VAP, or that vertical pathogen transmission is more frequent than horizontal transmission among patients.
Material and methods: The retrospective observational study comprised patients meeting the clinical criteria of VAP. Controls were patients requiring mechanical ventilation for more than 48 hours with no signs of VAP. Each group was compared according to previous ATB application. Endotracheal aspirate (ETA) samples and oropharyngeal swabs (OS) were taken from all patients. Cultured isolates were identified using standard microbiological techniques. Antimicrobial susceptibility testing (AST) was performed according to the European Committee on Antimicrobial Susceptibility Testing (EUCAST) guidelines. Their clonal relationship was determined by comparing their macrorestriction profiles obtained by pulsed-field gel electrophoresis. Restriction profiles reaching 97% similarity were considered identical.
Results: A total of 104 patients were included. The criteria for VAP were met by 47 patients (14 patients with previous ATB use, 33 patients without the therapy). The frequency of VAP was lower in the group with previous antibiotic therapy (36%) compared to the group without the therapy (51%; p = 0.159). The most frequently identified pathogens were enterobacteria, including strains producing broad-spectrum beta-lactamases (ESBL, AmpC), and Pseudomonas aeruginosa strains. There was no statistically significant difference in the proportion of MDR pathogens between the group with previous antibiotic therapy (71%) and the group without the therapy (64%; p = 0.742). The 30-day mortality was lower in the group with previous antibiotic therapy, but no statistical significance was shown (p = 0.688). The clonality study comprised 20 patients whose VAP was caused by Klebsiella spp. and Escherichia spp. strains. A total of 49 bacterial isolates were collected. In 75% of cases, bacterial pathogens causing VAP had 9 been isolated from the upper airways before the onset of VAP or bacterial strains identified in upper airway and ETA samples showed identical restriction profiles in the patient.
Conclusions: Previous antibiotic therapy is associated with lower incidence of VAP and slightly higher frequency of MDR bacterial strains. The 30-day mortality was slightly lower in VAP patients with previous antibiotic therapy. The wide variety of bacterial pathogens isolated from different patients is suggestive of predominantly endogenous etiology of VAP infection. The knowledge of previous antibiotic therapy and microbiology test results may contribute to early modification of VAP therapy and more favorable 30-day mortality.
