Obsah předmětu
Cardiopulmonary Resuscitation: Advanced Life Support (ALS), Pediatric Advanced Life Support (PALS). Manual defibrillator. Practice on high-fidelity simulator. Anesthesia: Pre-anesthetic evaluation, types of anesthesia, pharmacological agents, procedural course, and complications. Airway management. Patient monitoring during and after anesthesia. Perioperative care. Management of acute and chronic pain. Simulator training and practical experience in anesthesiology department. Intensive Care: Aspects of intensive care (ICU protocols, monitoring, patient stabilization, nosocomial infection issues). Nutrition. Respiratory insufficiency, Acute Respiratory Distress Syndrome (ARDS), and fundamentals of mechanical ventilation. Shock states and sepsis. Organ donation program. Brain death and its determination. Ethics in anesthesiology and intensive care, and palliative care. Practical training in both acute and chronic intensive care units.

Výstupy z učení
The objectives of the course are: 1. To teach students to provide basic and advanced cardiopulmonary resuscitation, including acquiring the skills to maintain airway patency by using simple devices or performing other life-saving procedures. 2. To instruct students in the assessment of patients and their preparation prior to anesthesia. 3. To familiarize students with fundamental anesthesia management procedures using available anesthetic techniques. 4. To teach students to adequately treat acute and chronic pain. 5. To familiarize students with the organisation of intensive care, the spectrum of care provided and ethical considerations. 6. To introduce students to the fundamentals of mechanical ventilation, infusion therapy, nutritional intervention and other supportive techniques. 7. To teach students to report information about current health status, response to treatment to date and suggest further therapeutic approach or limitation of therapy
After completing the course students will: 1. Acquire knowledge of recommended protocols for Advanced Life Support (ALS) and Paediatric Advanced Life Support (PALS), develop practical proficiency in these procedures, and enhance their skills in Basic Life Support (BLS) and Paediatric Basic Life Support (PBLS). Familiarize themselves with Crisis Resource Management (CRM) theory, developing both technical and non-technical skills. 2. Be able to assess patient anesthesiological risk using the ASA classification, evaluate difficult intubation risk and Post-Operative Nausea and Vomiting (PONV) risk, and become acquainted with recommended PONV prevention protocols. Be capable of discussing various general and regional anesthesia techniques for adults and children. Become familiar with anesthesia administration for both surgical and diagnostic procedures. Know basic airway management tools (including for difficult airways). Understand fundamental pharmacological agents used in anesthesia, their indications, contraindications, and adverse effects. Be able to enumerate, recognize, and initiate treatment for the most critical anesthesia-related complications. Understand the basic components of anesthetic machines, monitors, and other anesthetic equipment (including ultrasound). Be aware of environmental risks associated with inhalational anesthetics and methods to minimize their environmental impact. Understand post-anesthesia patient monitoring and the use of recovery rooms. 3. Comprehend fundamental aspects of acute and chronic pain, including basic pharmacological and non-pharmacological treatment approaches. Be able to assess pain intensity using Visual Analogue Scale (VAS) and Numerical Rating Scale (NRS). Be introduced to the basics of regional anesthesia, including relevant anatomy for regional techniques in surgery and pain management, commonly used drugs, and frequently employed ultrasound-guided blockade techniques. 4. Be able to discuss indications for admitting patients to intensive care, knowing basic admission criteria and contraindications. Become familiar with standard and advanced patient monitoring in the ICU. Learn to examine unconscious patients. Understand the most important causes of acute and chronic respiratory insufficiency, and be able to discuss clinical presentation, treatment, and prognosis. Know the recommended protocols for diagnosing and treating sepsis, and understand the basics of rational antibiotic therapy. Be able to diagnose shock, and discuss various types of shock, their clinical presentation, and treatment. Be introduced to the basics of nutrition in critical care, including enteral and parenteral nutrition, indications, contraindications, and nutritional goals. Be able to assess nutritional status and patient nutritional risk. Understand fundamental homeostatic disorders - acid-base imbalances, hydration disorders, and their causes, and be able to initiate their treatment. Be familiar with organ donation programs and the role of intensive care medicine in this field, including brain death determination and issues related to donation after circulatory death and living donation. Be acquainted with ethical issues in intensive care medicine and the basics of palliative care, end-of-life decisions in the context of intensive care medicine, and be able to discuss these topics.
Předpoklady
Knowledge of anatomy, physiology, and pathophysiology of all organ systems, particularly respiratory, cardiovascular, and nervous systems, including fundamental understanding of acid-base and electrolyte balance, and homeostasis. Comprehensive knowledge of pharmacology, specifically medications commonly utilized in anesthesia and intensive care (anesthetics, neuromuscular blocking agents, vasopressors, inotropes, sedatives, analgesics), including drug mechanisms of action, pharmacodynamics, and pharmacokinetics. Proficiency in understanding common infections, including nosocomial infections, sepsis pathogens, and principles of rational antibiotic therapy. Fundamental knowledge of immunology. Competency in internal medicine and surgical propedeutics, including practical skills (patient examination, vital sign monitoring, ECG). Capability to interpret basic radiological, CT, and ultrasound findings. Basic comprehension of ethical principles and communication. Knowledge and skills in providing basic life support and cardiopulmonary resuscitation for both pediatric and adult patients. General surgical knowledge (types of surgical procedures, preoperative patient preparation). Understanding of basic sterile techniques and infection prevention. Ability to interpret fundamental laboratory results.
